HIPAA & Confidentiality

As a licensed therapist and social worker, I am legally and ethically required to maintain the confidentiality of everything you share in therapy. Your information will not be disclosed to anyone without your written consent, except in the following legally required situations:

• If there is reason to believe you are at serious risk of harming yourself or others
• If there is suspected abuse or neglect of a child, elderly, or dependent adult
• If required by court order or subpoena
• If you provide written authorization to share specific information with another party (i.e., a physician, family member, or school)

Your health information is stored securely using HIPAA-compliant systems for notes, communication, and telehealth.

Informed Consent

Before starting therapy, you will receive and sign an Informed Consent for Treatment form. This document explains:

• What therapy is and what to expect
• Your rights as a client
• My responsibilities as your therapist
• Session fees, payment policies, and cancellation terms
• How to contact me between sessions (and when I may be unavailable)

You’ll also receive a Notice of Privacy Practices (also below), which outlines your HIPAA rights, including how your health information may be used and shared.

All forms are provided electronically and can be reviewed at any time. If you have questions about your privacy or the consent process, I’m happy to talk through them with you.
